AbleCare has group homes and AFL’s (Alternative Family Living).
What is the difference between a group home and AFL (Alternative Family Living)?
A group home is a home where 2-4 individuals live. Each of those individuals receives 24 hours services and supports. Multiple Staff rotates in 8 hour shifts per 24 hours.
An AFL is when an identified Person or Family opens their personal home and invites an individual that requires services and supports to live with them. The “primary staff” is the home owner. The benefit is fewer staff interpreting the Person Centered Plan to allow for more consistent service delivery in this type of therapeutic setting.

Medicaid-CAP-MR/DD State Funding-Contracts with specific County Local Management Entity’s
I need help with someone I care for. How do I go about getting your services?
Contact the Local Management Entity (use to be called the Mental Health Center) in your county. They can assist you with linking with a Case Manager. Once you have a Case Manager, that position will assist you with obtaining funding that you are eligible for.
